RCA2 Session 2 - Finding & Mapping the Facts, Causal Statements to Facilitate Action
In the second session of the series, we will determine how to best find event facts and then map them when an event of harm occurs. We will also share how to write effective causal statements which will allow you to develop effective corrective actions.
